Your final step before our first Awaken Spiritual Mentorship call (eee!) is to complete your questionnaire. This is due one week before your first call.

The intention of the questionnaire is to begin to set the energetic foundation for our sacred time together to create the highest version of transformation and discovery as you rise into the next layer of your truest, most divine self.

Confidentiality is the foundation of our relationship. Everything shared in our conversations remains private. Your openness allows me to support you more fully, so please take your time to answer the following questions in detail. This will help us align deeply on your vision, goals, and the path forward. 

Please take your time to reflect and provide long-form, in-depth answers. Your insights will shape our journey and allow for the deepest transformation possible. You can also transfer the questions to a piece of paper or document on your computer, so you can really take your time without feeling you have to complete it in one sitting. This way you can also come back after deeper reflection to add more until it feels complete.
